Our 16-18 transition homes are designed to support those stepping down from secure services, including hospital – as well as those who are ready for more independent living following residential homes.


We are also able to accept those who are new to care who’s needs can be met within a supported living environment. With experience in working with individuals with mental health difficulties and learning disabilities we are able to offer both placement opportunities.

  • Promote positive health, wellbeing and lifestyle choices.Daily living skills, cleaning and shopping

  • Accessing education, training and employment
  • Health & well being discussions with key worker around issues such as drugs, safe sex, diet etc.
  • Preparation for move to independent living.
  • Ensuring all health checks/medical appointments are kept.
  • An Activities Programme is available.
  • Work with families and partners in order to establish a network of support 
  • One-to-One specialist work from the Therapeutic Support Team, e.g. Anger Management, Sexual Health, Drug & Alcohol Use, Parenting Skills, Self Esteem, Social Skills etc.

The service is time-allocated (timescales are to be agreed at the Pre-placement Planning meeting and reviewed at regular intervals) and young people will be expected to move on to the Step-On provision or full independent living.
